Client and Patient Information

Dr. Thibodeaux sees patients by appointment.  Making an appointment allows us to schedule time to examine your pet and discuss our finidings with you.  It also keeps you and others from waiting and helps keep us on schedule.  If someone arrives with an emergency without an appointment we will do our best to accomodate them, assessing the patient as soon as possible.

Please call our office to schedule an appointment at (337) 893-0187 or fill out our Appointment Request form here.

Click here to download our new client and patient form. You can fill it out and bring it with you for your appointment, or scan and email it to us at ThibodeauxAnimalHospital@TAH.glacoxmail.com prior to your appointment.

You can come in anytime during regular office hours for refills of prescription medications and other pet supplies. Click here to request a refill of previously prescribed medications.

We also offer drop off services for your pet. You can drop your pet off anytime during office hours and pick it up later at a convenient time.  Sometimes this works out best for both you and us to care for your pet.  If we need more time with your pet for further diagnostics (blood work or xrays, etc.) we may ask you to leave him/her with us for the day.  When dropping off, please make sure you leave appropriate contact information so we may get back with you if needed.

Click here to download our Pet Visit Questionaire to answer some questions about your pet's condition before you arrive for your appointment.  You can return it to us at your appointment, by fax or email as above.

Click here to download our Pre-Surgical Questionnaire which will help us to evaluate your pet's health prior to any surgical or anesthetic procedure.
